Completion of the following form and a paid deposit will enter you for a chance to be selected for a tattoo appointment.

Spooky Empire 2026 will be Friday October 30th - Sunday November 1st, at the Hyatt Regency Orlando: https://spookyempire.com

This deposit goes towards the final price of the tattoo. Separate Spooky Empire admission tickets are required to enter the convention.
